Purpose
miaminos-reviewed vendor evidence.
Notice version 1: July 30, 2026.
Responses are collected to support a documented, miaminos-reviewed Vendor Intelligence assessment. They may be used to locate public policies, verify testing-report practices, understand shipping and claims controls, and send factual verification or correction questions.
The form does not request legal-identity or ownership records, personal addresses, dates of birth, government identification, previous business names, customer information, patient information, or confidential laboratory credentials.
Answer visibility
Private responses and profile-eligible answers stay distinct.
Internal review only. Used by authorized miaminos reviewers for due diligence, verification, correction, and audit history. It is not public directory copy.
Vendor Intelligence profile eligible. An eligible answer may be paraphrased in a future profile available to signed-in members and identified as vendor-provided. This applies only if the vendor is listed and a miaminos reviewer verifies and approves the information.
Visibility is selected separately for each answer. The role or team, shared business inbox, and authorization attestation remain internal. Do not provide an individual representative’s name, title, or direct email address.
Evidence and attribution
Vendor-provided provenance and verification
Questionnaire responses are labeled internally as vendor-provided. Submission does not make an answer independently verified. miaminos may compare responses with public vendor pages, official regulator or registry records, and a named laboratory’s authorized verification method. Reviewers may contact the role-based response address for clarification.
Member-profile summaries, if any, will distinguish vendor-provided statements from independently checked evidence and will include review dates and material limitations.
Private links and version history
Drafts, submissions, and amendments
A draft can be saved and resumed through the same private link until the invitation expires or is revoked. The link is reusable during that period; it is not a one-time-use token. A final submission is preserved as an immutable version. Later changes create an amendment; they do not silently replace the earlier submitted record.
Do not forward the private link. Anyone who possesses it may be able to view or change the working draft until the invitation is expired or revoked.
Storage and safeguards
Retention and security
In the initial private-QA version, drafts, submitted versions, amendments, attestations, and related audit records are stored in the protected application database. Expiry or revocation prevents further use of the private link but does not automatically delete the retained record. Automated retention and deletion are not yet connected.
No external invitation should be issued until miaminos approves and documents an operational retention period, deletion process, exception handling, and responsible owner. This notice must be updated to state that approved schedule before external use.
This initial questionnaire does not store IP addresses or browser user-agent strings with vendor responses. Invite, draft, and submission timestamps may be used to apply request controls and preserve the review record. Access to questionnaire records is restricted to authorized reviewers.
Vendor support
Corrections, withdrawal, and contact
A vendor may request correction of a factual error, provide updated evidence, or ask that a working questionnaire be withdrawn. Some audit or submitted-version records may need to be retained to preserve the integrity of the review history.
Contact vendors@miaminos.com and include the vendor display name and a description of the requested correction. Do not email customer, patient, payment, or government-identification information.
For website-wide privacy practices and privacy-request options for Canada and the United States, read the miaminos Privacy Notice.
